At EYFC, we are ONE team - Eastlake Panthers! In keeping with our mission, we strive to help every child learn, and thrive in a safe environment. Any and all team or placement requests must be made in writing and sent directly to the board member in charge of eligibility for that particular division. All requests will be reviewed and honored when possible. It is the boards position that players and cheerleaders are registered to participate as Eastlake Panthers, NOT to participate on a particular team or play for a particular coach.

(includes practice uniforms, game uniforms, referee fees, insurance, 1 day cheer camp for cheerleaders, trophy basic picture packet,assessment fees, use of football gear)
Flag Football - $230
Tackle Football - $280
All Cheer - $185
Cheer uniform extra, The 2010 cheer uniform is still being finalized and no final price is available yet. We are trying to keep costs to parents down and the uniform price will not be higher than last year's price of $217.00

Open Registration
Saturday, April 14 6pm - 8pm @ Eastlake High School
Wednesday, April 17 9am - 12pm @ Gonzalez Sports Academy
Saturday, May 1 9am - 12pm @ Eastlake High School


** Parent/Guardian AND Child must attend registration **

Required Documents - bring these to registration

* 3 copies player contract (link - see attachment) - if you use a digital photo for the contact
picture it MUST be a clear image, head shot only. See player contract, pictures can be
downloaded from your computer directly to the contract. Otherwise please bring 3 - 1 x
1 inch head shots to registration. (Please note each copy of the contract must have a picture of the child)

* 2 different forms of proof of residency. These can be copies, but we will need to keep them (just 1 copy each, utility bill, rental agreement, etc.)

* Registration Fees in full (cash, money order, cashiers or bank check, or personal check)

* Refund policy (link - attached) 1 copy, signed

* Do NOT bring your child's report card to registration

Required Documents - must be received by June 30

* Report Card - 2 copies of the end of the year report card, which your child will receive in June 2010

* Physical Form (link - attached) - this must be completed by a physician (NOT A CHIROPRACTOR), physical must have taken place in 2010.
